Social Worker - LCSW Profession: Social Work Specialty: Care Coordinator Social Worker (Discharge Planning) Duration: 13 weeks Shift: 8H Days Hours per Shift: 8 hours Experience: Minimum 2 years of experience in Acute Inpatient Medical Case Management. License: Active License - LMSW or LCSW and Active State License Certifications: BLS Certification required Must-Have: Minimum 2 years of Acute Inpatient Medical Case Management experience, including at least 1 year in a large busy hospital Emergency Department with 500+ beds. Recent acute care discharge planning experience for patients with a wide range of medical diagnoses. Extensive experience in psychosocial assessments, telephonic management, and utilization management. Strong computer skills, proficient in EPIC charting. Description: We are looking for a highly experienced Clinical Care Coordinator. Candidates should have experience in acute inpatient case management and discharge planning. Training on the unit will be minimal, and candidates must be able to pick up quickly and work efficiently. The case load will consist of 15-20 patients per day. Consultations with the team will be available as needed. The facility reserves beds for complex placements, but specific criteria must be met for assistance. Candidates will receive a variety of complex patients due to the facility's status as a trauma hospital. Candidates must be able to dress in any color scrubs or business professional attire.
