Your Role Includes:
- Evaluating residents and creating individualized treatment plans
- Supporting residents in improving mobility, balance, strength, and functional independence
- Collaborating with interdisciplinary teams for resident-centered care
- Documenting progress according to regulatory and company standards
What We're Looking For:
- Active Wisconsin Physical Therapist license
- Strong clinical, communication, and documentation skills
- A passion for working with older adults in assisted living
- Dependability, adaptability, and a team-oriented mindset

- Graduate of an accredited school with a minimum of a bachelor's degree in Physical Therapy.
- Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist.
