Social Worker - Acute Care Case Management

We are seeking a dedicated Social Worker for our Acute Care Case Management team. This position involves working with a diverse patient population to facilitate discharge planning and case management in a fast-paced healthcare environment.

Responsibilities

• Conduct comprehensive biopyschosocial assessments for patients.
• Develop and implement effective discharge plans in collaboration with a multidisciplinary team.
• Coordinate post-acute care services, including home care and equipment needs.
• Provide crisis intervention and support for patients and their families throughout the discharge process.
• Document all aspects of care in compliance with internal and external guidelines.
• Work closely with insurance case managers and other healthcare professionals for optimal patient outcomes.

Required Experience & Certifications

• Minimum of 2 years of experience in acute care case management or discharge planning.
• Must hold a valid MSW licensure in California; pending licenses are not accepted.
• Experience with electronic health records, specifically Epic.
• Strong knowledge of community resources and healthcare regulations.
• Experience in working with diverse populations and demonstrating cultural competency.

Schedule

This is a Day Shift position with a 5x8 hour schedule, from 08:00 to 16:30.
