Responsibilities: • Perform in-home patient assessments and develop individualized care plans. • Provide skilled nursing care including wound care, medication management, and health education. • Monitor patient progress and adjust care plans as needed. • Coordinate services with physicians, therapists, social workers, and caregivers. • Document assessments, interventions, and patient outcomes accurately and timely. • Educate patients and families on disease management, safety, and home care instructions. • Ensure compliance with regulatory, agency, and quality standards.
