Job Description
Title: LPN - Long Term Care

Schedule:
- Day shift
- 6am-6pm or 10am-10pm
- Including every other weekend

Assignment Details:
- Contract Length: 8 weeks
- Guaranteed Hours: 36 per week

Requirements:
- Must be licensed in New York
- 1+ years of experience required
- BLS certification

Roles & Responsibilities:
- Provide nursing care to patients in a long-term care setting.
- Assist with daily living activities, including personal hygiene and mobility.
- Monitor and record patient vital signs and health status.
- Administer medications as prescribed and manage treatment plans.
- Collaborate with healthcare team members to ensure quality care.
- Document patient care and report changes in condition to the supervising nurse.

Compliance / Notes:
- Must be willing to float to different units as needed.
