Job Summary

We are seeking a skilled Physical Therapy Assistant (PTA) to provide high-quality, patient-centered physical therapy and rehabilitation services under the supervision of a Registered Physical Therapist (RPT).

This position is based in a Skilled Nursing Facility (SNF) and sub-acute inpatient rehabilitation setting , providing therapeutic care to geriatric and post-operative patients. The PTA will implement established treatment plans, support functional mobility and recovery, monitor patient progress, and maintain accurate clinical documentation.

Key Responsibilities

• Provide physical therapy treatments and therapeutic interventions according to the plan of care established by the supervising Physical Therapist .
• Assist patients with gait training, therapeutic exercises, neuromuscular re-education, transfers, and functional mobility .
• Apply appropriate therapeutic modalities, including heat, cold, ultrasound, and electrical stimulation , as directed by the treatment plan.
• Monitor patient responses to treatment and identify changes in functional or clinical status.
• Document treatments, patient progress, responses, and outcomes accurately in the NetHealth (Rehab Optima) EMR system .
• Communicate patient progress, concerns, and significant changes to the supervising Physical Therapist.
• Educate patients and caregivers on therapeutic exercises, transfers, mobility techniques, self-care, and home safety .
• Maintain compliance with facility policies, clinical standards, and applicable professional requirements.

Mandatory Requirements

• License: Active and unrestricted California Physical Therapist Assistant (PTA) license .
• Experience: 1-2 years of recent clinical experience as a Physical Therapy Assistant.
• Certification: Current Basic Life Support (BLS/BCLS) certification.

Preferred Qualifications

• Experience with NetHealth (Rehab Optima) documentation software.
• Previous experience in a Skilled Nursing Facility (SNF), sub-acute rehabilitation, inpatient rehabilitation, or geriatric care setting.
• Strong communication, patient-care, documentation, and clinical skills.
