Job Summary
Evaluate, develop, and implement individualized treatment plans to help patients achieve their functional goals and safely return to daily activities. Collaborate with physicians and the rehabilitation team to deliver high-quality, evidence-based outpatient therapy.

Responsibilities

Perform comprehensive patient evaluations and develop individualized treatment plans
Deliver skilled physical therapy services to maximize functional outcomes
Educate patients and families on treatment plans and home exercise programs
Document evaluations, treatment sessions, progress notes, and discharge plans accurately and timely
Collaborate with physicians and the interdisciplinary care team
Supervise and mentor students as assigned
Maintain compliance with state practice standards and clinic policies
Participate in professional development and quality improvement initiatives

Required Qualifications

Degree in Physical Therapy from an accredited program
Active Physical Therapist license in the state of practice
1+ year of outpatient clinical experience
CPR Certification

Preferred Qualifications

Previous travel assignment experience
Previous Select Medical assignment experience
