Nurse Practitioner OpportunityLifeBridge Health is seeking a Nurse Practitioner to join our New Hope Treatment Substance Use Disorder program at Grace Medical Center. NP candidates required to have training and experience with substance use treatment.The qualified Nurse Practitioner is responsible for patient intakes, assessment and dose management in collaboration with the Medical Director. Under this program, New Hope offers many types of treatment and services to patients who struggle with the disease of addiction. All services are certified by the State of Maryland and are CARF accredited. The treatment program is for adult patients (18+).Primary medical care is also offered to all patients enrolled in our treatment program. Our care team helps manage acute and chronic illnesses like hypertension and diabetes, collaborates with local pharmacies to provide vaccinations and preventative care including women's health services. In addition, patients are provided access to education and counseling for healthy living as well as referrals to specialists. Behavioral Health Home services are available to Medicaid patients enrolled in this treatment program. In Behavioral Health Home is to bridge the gap between behavioral health and physical health, helping patients with complex behavioral health needs receive consistent, coordinated medical care.Position Details· Full-time outpatient, hospital-employed position.· No On-Call responsibilities. No regular weekend or evening hours.· The schedule is as follows:o Monday, Tuesday & Thursday, 7:00-5:30pm (an 8 hour shift within this timeframe)o Wednesday & Friday, 6:00am-2:30pm.· Based onsite at the New Hope Treatment Center.· Manages the Behavioral Health Home Program, in conjunction with the administrative staff.· Assesses, diagnoses, prescribes and monitors Substance Use Disorder, (SUD) medications, educates, counsels and refers patients as needed.· Perform primary care duties such as completing patient referrals, taking vitals, prescribing medications, managing chronic diseases and conditions, designing preventive care plans, diagnosing conditions, ordering and interpreting medical testing, providing patient education, coordinating care, providing health counseling, maintaining documentation, performing patient follow up, advocating for patients and collaborating with other healthcare professionals· Responsible for Behavioral Health Home (BHH) to include, a Care Coordinator, and a RN.Benefits· Compensation Range: $50 - $70/hour contingent on experience· Full benefits - medical, dental and vision plans· Medical malpractice insurance· Life insurance· 403b retirement plan· Professional dues reimbursement· HRSA-approved student loan repayment program· Tuition reimbursement· CME annual allowance· Paid Time Off (PTO) and paid holidaysRequirements· Master's degree and Advanced Practice Nurse, Nurse Practitioner license (APN, APRN, or NP). Certification with AANP or AANC. Must be a Family Nurse Practitioner or an Adult Nurse Practitioner.· Must also be certified to perform primary care duties as a NP.· Maryland DEA & CDS license and current CPR license.· 1 - 2 years' NP experience in substance use treatment required.· Excellent interpersonal and human relations skills· Demonstrated computer/EMR proficiency.
