Physical Therapist

Schedule: Days: 8am-5:30pm (Schedule can be flexible based on patient's availability.) Weekends: One weekend

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• One year of Home Health experience

Roles & Responsibilities:

• Evaluate and treat patients using medically prescribed physical therapy programs.
• Document clinical findings, observations, interventions, goals, and discharge plans.
• Supervise Physical Therapy Assistants as needed.
• Conduct initial admission and evaluation visits.
• Report clinical information and adhere to established timelines for documentation.
• Perform additional duties aligned with skills and training.
