About PACE Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health ( is a non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center is entrepreneurial in nature and focuses on innovations and bringing new processes to the traditional PACE environment. We're looking for passionate healthcare providers who want to be part of a team dedicated to enable seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care. One Team One Mission Position Summary: The Clinical Supervisor provides clinical leadership, supervision, and operational oversight for the organization's Nurse Practitioners (NPs) and Physician Assistants (PAs) within the Program of All-Inclusive Care for the Elderly (PACE). This role serves as the primary liaison between the Medical Director and the Providers, ensuring consistent clinical practice, effective communication, quality care, and compliance with regulatory requirements and organizational standards. The Clinical Supervisor provides direct supervision, coaching, mentoring, performance management, scheduling support, and professional development for Providers while maintaining a reduced panel of PACE participants as assigned. Working collaboratively with the Medical Director and interdisciplinary team (IDT), this position promotes high-quality participant-centered care, continuous quality improvement, and operational excellence. Essential Functions Leadership and Supervision Directly supervise Providers (Nurse Practitioners and Physician Assistants). Serve as the primary communication link between the Medical Director and Providers. Provide clinical guidance, mentoring, coaching, and support to Providers. Conduct regular one-on-one meetings with providers to review performance, workload, and professional development. Participate in recruitment, interviewing, onboarding, orientation, and retention of Providers. Complete performance evaluations and provide timely coaching and corrective action when appropriate. Assist with provider scheduling, coverage planning, and workload balancing. Foster collaboration, accountability, and a positive team culture. Assist in resolving provider concerns and operational issues. Escalate significant clinical or operational issues to the Medical Director as appropriate. Clinical Operations Promote consistent clinical practice standards among Providers. Support implementation of evidence-based clinical guidelines and PACE standards of care. Monitor documentation quality, timely completion of medical records, and regulatory compliance. Collaborate with leadership to improve clinical workflows and operational efficiency. Participate in development and implementation of policies, procedures, and clinical protocols. Assist with provider education regarding regulatory changes and organizational initiatives. Quality Improvement Participate in Quality Assurance and Performance Improvement (QAPI) activities. Monitor clinical quality indicators and assist with improvement initiatives. Participate in chart audits and documentation reviews. Identify opportunities to improve participant outcomes, provider performance, and clinical efficiency. Assist with preparation for regulatory surveys and audits Direct Participant Care Maintain a reduced participant panel based on organizational needs. Participate in on-call rotation with other Providers Provide comprehensive primary care services consistent with the PACE model. Perform participant assessments, diagnoses, treatment planning, and follow-up. Participate as an active member of the interdisciplinary team (IDT). Develop and update individualized plans of care. Coordinate care across disciplines and community providers. Respond to urgent participant needs as required. Collaboration Partner closely with the Medical Director regarding clinical initiatives, provider performance, and organizational priorities. Collaborate with nursing, rehabilitation, social work, home care, behavioral health, and other departments to ensure coordinated participant care. Participate in leadership meetings and organizational committees as assigned. Support implementation of strategic initiatives affecting clinical operations. Protect privacy and maintain confidentiality of all company procedures and information about employees, participants and families Follow GMWP policies and procedures and participate in any required QI activities, staff training and meetings Maintain professional licensure and certifications. Act only within the scope of practice. Education and Certification: Master's or Doctoral degree from accredited Nurse Practitioner or Physician Assistant program Current California Nurse Practitioner license or Physician Assistant license. Current DEA registration. Current Basic Life Support (BLS) and CPR training. Current driver's license and proof of auto insurance. Clean driving record. Skills and Experience: Minimum five years of clinical experience as an NP or PA doing adult chronic disease management and functioning in PCP role with or without MD oversight. Specific work experience with the frail, elderly population is preferred. Minimum two years of leadership, supervisory, or lead provider experience preferred. Current knowledge of chronic care/geriatric issues and best practices, experience with deprescribing and palliative care would be a bonus Ability to build strong working relationships with other providers and an interdisciplinary team (IDT) Creative and enthusiastic commitment to the "triple aim" – dedication to improving experience, health, and value for PACE participants Ability to process information accurately and timely, and able to learn new EMR and health tech and communication platforms quickly Excellent clinical, organizational, and communication skills in settings with frail, elderly participants, their families. Experience with having difficult goals of care and "What Matters" conversations would be preferred. Working knowledge of PACE regulations would be a bonus but not required. Medical Clearance: Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
