By assigning higher point values, we can directly tie your compensation to the work you perform and time spent outside of a visit./ppJob Purpose:/ppAssist the patient and patient caregiver with personal hygiene needs to increase the level of comfort of the patient./ppKey Responsibilities:/pp Provides competent nursing care to home health patients according to state and industry and profession./pp Demonstrates knowledge of home care for specific patient care skills/pp Helps maintain continuity of care by collaborating with the interdisciplinary team/pp Obtain vital signs of patients as assigned./pp Follow procedures for handling medical emergencies or other incidents that affect the delivery of service./pp Report patient progress and problems to supervisory personnel./pp Observe and report any changes in condition, including skin integrity./pp Provide comfort care related to turning and positioning./pp Maintain a safe, clean and comfortable environment for the patient./pp Assist with meal preparation and related tasks./pp Assist with patient transfer, ambulation and exercise of patients./pp Provide personal hygiene to patients, including bathing, shaving, oral hygiene, skin care, dressing, toileting, and hair care./pp Maintain accurate and current records of visits./pp Communicate and document tasks in a timely and accurate manner./pp Ability to be self-directed, prioritize, manage time appropriately, and meet outcomes and strategic goals by targeted deadlines./ppAs a member of our team, clinicians will have access to top-of-the-market pay structures with unlimited income potential, progressive benefit plan, mileage reimbursement, opportunity for career growth, additional pay incentives, and flexible schedules plus a great team environment that reflects our commitment to caring for our 16,000 partners./ppMinimum Education Required:/pp Certificate of completion of a state recognized Home Care Aide Training Program and supervised clinical practice./ppMinimum Experience Required:/pp One (1) years of experience working with adults in a healthcare setting./ppMinimum Licensure/Certification Required by Law:/pp Current registration in the state Nurse Aide Registry as a Certified Nurse Aide in state of practice./pp Current CPR certification./pp Valid drivers license and automobile liability insurance./ppAdditional Qualifications: (Preferred qualifications)/pp Home Health experience is desirable./ppFamily Makes Us Stronger.
