Job: Assessment Therapist Job Details Job Location: Chicago Main - Chicago, IL 60639 Position Type: Full Time Education Level: 4 Year Degree Salary Range: $40.00 - $40.00 Hourly Travel Percentage: Up to 50% Job Shift: Swing Job Category: Nonprofit - Social Services Job Overview The Assessment Therapist position performs a variety of duties both clinical and clerical in nature. The position requires skill planning, organizing and coordinating the delivery of consumer care. This position requires completing IM+CANS assessments and screening with consumers in a face-to-face environment before entering services with CTS. This role also requires excellent customer service and friendliness when interacting with consumers as this role is one of the first few people they meet from CTS. Clinical Responsibilities and Duties Complete IM+CANS mental heath assessments and behavioral screening (initial and updates), which involves collection of consumer mental health and trauma history. Maintain confidentiality of records relating to clients' treatment. Maintain a minimum of an hourly billable caseload of 25 per week. Determine LOCUS score in order to determine level of care. Monitor the mental and physical health needs of the clients and report any report concerns to supervisor. Cooperatively develop individual goal plan and interventions with consumers and treatment team, emphasizing person-centered care. Receives and coordinates all referrals to include notification of insurance providers for prior authorization and services covered. Complete intakes within 24 hours of seeing consumer. Responsible for attending to all the rules and regulations of the agency, state, and federal statues, and attending rules of quality improvement, medical records, finance, etc. of the agency (including delivery of service to consumers under best practice models). Encourage clients to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships. Guide clients in the development of skills and strategies for dealing with their problems. Complete clinical documentation and submit using OnTarget database in a timely manner. Counsel clients and patients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes. Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations. Develop and implement treatment plans based on clinical experience and knowledge, and update as required. Meet with families, probation officers, police, and other interested parties in order to exchange necessary information during the treatment process. Refer clients, clients, or family members to community resources or to specialists as necessary. Counsel family members to assist them in understanding, dealing with, and supporting clients or patients. Evaluate the effectiveness of counseling programs and clients' progress in resolving identified problems and moving towards defined objectives with other staff members as part of the treatment team. Modify treatment activities and approaches as needed to comply with changes in clients' status. Learn about new developments in their field by reading professional literature, attending courses and seminars, and establishing and maintaining contact with other social service agencies. Discuss with individuals their plans for discharge with interest of supporting their future independence. Communication Communicate pertinent Consumer and Stakeholder information to other employees/team members to insure quality of care. Work with a team approach with other employees to ensure efficacy of care for ALL consumers. Report critical concerns and problems to supervisor in a timely manner and actively seek Supervisor’s input and assistance to resolving the concern. Follow chain of command when encountering problems or other areas of need and correct problems requested. Solicit and cooperate with assistance of all support staff. Actively participate in an appropriate manner with the decision-making process as “pro-active” team member and cooperate with team’s decision, whether programmatic or clinical. Provide coverage on‑site or as assigned by the needs of the agency including community events. Attend staff meetings, planned in‑service training and any other regular or assigned meetings. Responsible for any equipment assigned in order to perform duties. Facilitate flow of information, particularly as it affects the delivery of services to consumers. Shift variance to meet the needs of the clients, per supervisor approval. Manage own transportation, scheduling, staffing and service delivery concerns and cooperate with treatment team members in resolving concerns. Perform other required job duties that the supervisor and/or director require to maintain continuity of care for consumers and profitability for the agency. Participate in 90‑day and annual evaluations. Participant in state and local audits. Stay abreast of state changes and inform appropriate staff regarding new service definitions. Attend provider fairs, collaboratives, and state-sponsored meetings when applicable. When necessary, attend scheduled meetings, including, but not limited to, executive team meetings, state team meetings, school and treatment/plan team meetings. Follow up with consumers regarding integrity of services, if necessary Work with Clinical Director to assign consumers to therapists. Monitor timesheet weekly. Review OPT tracker for therapy hours and documentation schedule. Assist team lead supervisors Qualifications Bachelor’s Degree in a human service field required; Master’s Degree preferred. Possess and maintain full or provisional Licensure Status (LSW, LPC, LCSW, PLCSW, LPC, LPCA, LCASA eligible or Ph. D.), preferred, but not required. IM+CANS certification required upon hire (can be part of on the job training if not in possession). Must have adequate transportation and valid Driver’s License, registration and insurance to deliver intake services when necessary, in the community. Criminal records, fingerprinting, and HCWR check is a prerequisite for employment consideration. Trainings Obtained Must complete all agency initial orientation training and state mandated trainings. Must complete training and successfully pass Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) exam. #J-18808-Ljbffr CTS Health
