Role Overview: An Occupational Therapist provides personalized in-home and senior living care to older adults, promoting mobility, independence, and quality of life through evidence-based treatments. Key Responsibilities Conduct evaluations and develop individualized care plans Deliver therapy sessions to support wellness, longevity, and functional independence Document patient progress accurately in electronic medical records Collaborate with a multidisciplinary team including PTs, SLPs, and fitness specialists Educate patients, caregivers, and staff on safety and aging-in-place strategies Utilize telehealth and virtual care to extend access and flexibility Qualifications & Skills Valid Occupational Therapy license and degree from an accredited program Proficiency with computers and EMR systems Current CPR certification Physical & Other Requirements Ability to perform physically demanding tasks such as lifting, repositioning patients, and driving between locations. Must meet PPE requirements when necessary.
