Job Description
Schedule: Weekend requirement: Conflicting information in posting; please verify.
Assignment Details: Daily census: 255 patients; Visits per day: 15.
Requirements:
- Minimum 3 years of acute care case management experience.
- Active California RN license required at the time of submission (no pending or temporary licenses accepted).
- Required certification: BLS.
- Charting experience with EPIC highly preferred.
- Knowledge of utilization review (InterQual, Milliman), DRG/ICD-10 concepts, CMS, The Joint Commission/core measures, HIPAA, and OSHA.
- Two supervisor references from within the past year required at time of submission.
Responsibilities:
- Coordinate and manage patient care in an acute healthcare environment.
- Develop and implement discharge plans to ensure continuity of care.
- Conduct utilization review and ensure compliance with regulatory requirements.
- Collaborate with an interdisciplinary team to provide optimal patient outcomes.
- Assist in identifying and addressing barriers to care.
- Document patient interactions and care plans in compliance with healthcare regulations.
Compliance / Notes:
Mandatory flu vaccine required with no exemptions; COVID-19 vaccine not required. Max RTO is 2 weeks total; no RTO within the first 2 weeks of the assignment. Pre-start and orientation modules are required.
