Physical Therapist - Travel Contract

Minimum Requirements
* Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
* Active State License is Required to Start the Assignment
* BLS Certification May Be Required from AHA or ARC
* Home Health Experience
Assignment Details
* Shift Days: M-F
* Coverage area includes Deerfield south to the Ohio border and overflow coverage in the northern Downriver area.
* Expected travel may extend within an hour radius based on referral needs.
* EMR: Home Care Home Base
* On-call requirement: None
* Productivity expectation: 30 points per week
