Home Health Caregiver

Job Summary:

Wellhaven Home Care Charleston is seeking a compassionate and dedicated Home Health Caregiver to join our team of healthcare professionals in Myrtle Beach, South Carolina. As a Home Health Caregiver, you will have the opportunity to make a meaningful difference in the lives of our patients and their families by providing high-quality, personalized care and support.

Responsibilities:

• Provide personal care and assistance with daily living activities, such as bathing, dressing, and feeding, to patients in their homes

• Assist with medication management, including administering medications as prescribed by physicians

• Monitor and report changes in patients' conditions to healthcare providers

• Maintain accurate and detailed records of patient care and services provided

• Collaborate with healthcare teams to develop and implement individualized care plans

• Provide emotional support and companionship to patients and their families

• Perform light housekeeping and laundry tasks, as needed

Requirements:

• Home Health Caregiver certification or equivalent experience

• Ability to lift and transfer patients, as needed

• Excellent communication and interpersonal skills

• Ability to work independently and as part of a team

• Reliable transportation and valid driver's license (if applicable)
