Key Responsibilities Provides comprehensive patient care by interviewing, examining and treating clinic and/or hospital patients to meet their medical needs. Manage long‑term treatment plans for patients with chronic illness, ensuring regular follow‑ups and monitoring. Educates patients on wellness, prevention and early detection by providing materials and resources to patients and families. Determines level of urgency of follow‑up, referral, and/or consultation appointments based on examination and patient needs. Prescribes medical treatment and medications to patients. Ensures patient controlled‑substance contracts are completed for all patients being prescribed controlled substances, monitors use of medication and documents all results. Checks Vermont Prescription Monitoring System (VPMS) when appropriate. Orders studies, tests and ancillary services. Consults with Medical Director and other professionals on staff as needed regarding patient care, assessment, and education issues. Practices in a primary care setting at GCFH. Additionally, GCHF has a Same Day Clinic, and coverage may be required at times. Uses the electronic health record to document patient care and completes records according to Grace Cottage policy. Maintains confidentiality of patient information according GC policy and federal/state guidelines and regulations. May serve as a preceptor to medical students or other appropriate students per federal and state regulations. Adheres to Grace Cottage Medical Staff Bylaws, Rules and Regulations, Standards of Behavior, policies and procedures, and Granted Privileges. Participates in quality improvement, management, continuing education, and other patient care programs established by Grace Cottage. Assists in the resolution of complaints, requests and inquiries from patients. Qualifications Licensed to practice, or willing to license in Vermont. Graduate from accredited medical college with a medical degree and completion of a US residency program. Have a record that is free from current Medicare/Medicaid sanctions and not be listed on either the OIG List of Excluded Individuals/Entities or SAM. Possess a current, valid, unrestricted DEA number if applicable. Board Certified per Grace Cottage Bylaws. Insurability (malpractice).
