As a full-time member of our rehabilitation team, you will play a critical role in enhancing the quality of life for our patients through personalized physical therapy interventions. We prioritize diverse, multilingual communication to cater to our extensive patient demographic.Work EnvironmentYour role will be based in the homes of our patients in the Hayward area, allowing you to provide personalized care in a comfortable and familiar environment. Working hours are Monday to Friday, from 8 AM to 3 PM. You will join a supportive team of healthcare professionals who are passionate about rehabilitation and dedicated to collaborative practice, allowing you to effectively engage with multiple disciplines within our organization.Key ResponsibilitiesConduct comprehensive physical therapy evaluations for children and adults to establish a baseline for treatment.Design and implement individualized treatment plans that address the specific needs and goals of each patient.Continuously assess clinical signs and symptoms over the course of treatment, modifying plans as needed to align with patient progress.Utilize the Company's electronic record system to maintain compliance with documentation requirements, including progress notes and treatment summaries.Provide education for patients and their families regarding recovery processes and potential challenges, fostering a supportive therapeutic environment.Meet established utilization goals and contribute towards business objectives while maintaining excellent patient care standards.Collaborate effectively with colleagues from various disciplines to coordinate comprehensive, multifaceted care for patients receiving multiple services.Embody the core values of Body Balance Rehab, acting as an ambassador who champions our culture of collaboration and excellence.Patient Population / Client FocusYour role as a Physical Therapist will involve working with a wide range of patients, including children, adults, and elderly individuals, encompassing a variety of diagnoses and backgrounds, including diverse cultural and linguistic needs.
