Home Health Physical Therapist (PT)

Job Description Job Description This position is focused on delivering skilled physical therapy services in the home setting, helping patients improve mobility, reduce pain, restore function, and maintain independence following illness, injury, or surgery.
Key Responsibilities Perform comprehensive physical therapy evaluations and establish treatment plans.
Develop and implement individualized rehabilitation programs.
Monitor patient progress and modify treatment plans as needed.
Maintain accurate and timely clinical documentation.
Educate patients and families on treatment goals and home exercise programs.
Collaborate with physicians, nurses, and interdisciplinary care teams.
Follow infection control and patient safety standards.
Ideal Candidate Profile Licensed Physical Therapist in the state of practice.
Graduate of an accredited Physical Therapy program.
Current CPR certification.
Valid drivers license and auto insurance.
Home Health experience preferred.
HomeCare HomeBase experience preferred.
Strong documentation, communication, and patient education skills.
Physical Requirements Frequent standing, walking, kneeling, squatting, and patient mobility assistance.
Ability to lift, push, pull, or carry up to 100 pounds.
