Pay rate : $ 22 to $24 per hour

Responsibilities for this position include, but are not limited to:

  1+Yearof Long term Care Experience is Required
  Epic EMR is Preferred
  
  Certification required: BLS /CPR, CNA License
  
  Responsibilities:
  • Provide direct patient care, including bathing, grooming, dressing, toileting, and feeding assistance.
  • Monitor and document residents' vital signs, intake/output, and changes in condition.
  • Assist residents with ambulation, transfers, repositioning, and activities of daily living (ADLs).
  • Maintain resident safety, comfort, dignity, and infection-control standards.
  • Assist with meals, hydration, range-of-motion exercises, and restorative care activities.
  • Communicate residents' needs and condition changes to nurses and other healthcare team members.
