What You'll Do Conduct hospice admissions, eligibility evaluations, patient education, informed consents, and initial care planning. Provide case management support, including symptom management, care coordination, and clinical oversight as needed. Complete routine, PRN, and urgent patient visits to ensure continuity of care. Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and caregivers to provide holistic, patient-centered care. Educate patients and families throughout the hospice journey with compassion and confidence. Maintain timely, accurate EMR documentation while ensuring compliance with Medicare Conditions of Participation and hospice regulations. What You'll Bring Active Minnesota Registered Nurse (RN) license. Hospice experience preferred, or a strong clinical background in ICU, oncology, palliative care, home health, or other complex care settings. Excellent assessment skills, admissions experience, and confident clinical judgment. Strong communication and relationship-building skills with patients, families, and interdisciplinary teams. Highly organized with the ability to adapt in a fast-paced, field-based environment. Reliable transportation and willingness to travel throughout our service area.
