Care Manager RN Per Diem Day

Description Care Manager RN at Petaluma Valley Hospital in Petaluma, CA. This position is Per Diem and will work 8 hour, Day shifts. Utilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. Required Qualifications Upon hire: California Registered Nurse License 2 years of Acute care clinical or comparative experience. Preferred Qualifications Bachelor's degree in nursing. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
