Social Services Director

Social Services Director South Elgin Living and Rehab LLC, a skilled nursing facility dedicated to providing exceptional care and services to our residents, is seeking a compassionate and experienced Social Services Director to join our team. As a key member of our interdisciplinary team, you will play a vital role in ensuring the well‑being and quality of life of our residents. Responsibilities: Manage the coordination of services, programs, and data collection for residents, ensuring that their physical, emotional, and spiritual needs are met Provide oversight and guidance to staff, including social workers, therapists, and other support staff Develop and implement individualized care plans, working closely with interdisciplinary teams to ensure comprehensive care Facilitate skills groups, psycho-social groups, and other support groups to promote resident well‑being and engagement Collaborate with families to involve them in the care planning process and provide support and education Maintain accurate and timely documentation, adhering to regulatory requirements and facility standards Requirements: Master's degree in human services At least 1 year of experience in a skilled nursing facility, with a focus on crisis intervention, documentation, care planning, and support groups Proven ability to work in an interdisciplinary team environment and handle confidential data with professional discretion Nursing home experience: 2 years (Required) What We Offer: Competitive compensation package, including a salary of $ [insert salary] per year 401(k) plan Dental, health, and vision insurance Full-time schedule, 8-hour shifts, Monday to Friday Collaborative and supportive work environment Opportunities for professional growth and development #J-18808-Ljbffr
