RN, Pre-Transplant Coordinator

Job Summary: A registered professional nurse employed by Erlanger Health System is responsible for direct and/or indirect care of the patient from referral to transplantation at Erlanger Health System. Demonstrate leadership, communication skills, flexibility, and creativity while working with transplant patients and their families through all phases of transplantation. The pre transplant coordinator will need knowledge of chronic kidney disease, treatment options, and an understanding of the transplant process. The pre transplant coordinator applies policies and procedures to managing a case load of patients referred for transplant and functions as a resource for patients and staff about the transplantation process. Maintains communication with referring physician's pre and post-transplant. Is an active member of the multidisciplinary Transplant Team. Understanding of UNOS and CMS regulations. Active participate in transplant QAPI. Demonstrate problem-solving abilities as well as the ability to manage and/ or direct various personalities or educational levels in dynamic situations. Demonstrate computer skills and ability to work simultaneously in multiple databases as well as EMR. Exhibit the ability to work effectively and consistently in a group as part of a high function. Have the ability to work independently as well as a multidisciplinary team member. Demonstrate the ability to handle patient caseloads in an organized manner. Be committed to ongoing personal education in the specialty area. Ability to be cross-trained in transplant related areas.
