Home Health Registered Nurse (RN)

Location: Oahu, Hawaii

Assignment Duration: This is a 13-week contract position.

Schedule: Days, 8 hours; On-call for 6 shifts a month as needed, including weekends.

Job Description: The Home Health Registered Nurse provides excellent patient care by collaborating with nurses, physicians, and multidisciplinary teams to develop and implement patient care plans. This role focuses on optimizing patient outcomes by acting as a liaison between patients, families, and healthcare providers.

Responsibilities:

• Lead coordination of patient care with care team disciplines, ensuring appropriateness and timeliness of care.
• Monitor and revise interdisciplinary care plans consistent with patients' clinical courses and continuing care needs.
• Discuss resource utilization and treatment plans with physicians, focusing on complex patient cases.
• Collaborate with acute care case managers to maintain the continuity of care.
• Coordinate patient transfers to tertiary centers, ensuring necessary information is shared for ongoing treatment.
• Collect data for regulatory and accreditation compliance.
• Conduct studies on frequent emergency department visitors to analyze previous admissions.

Required Experience / Certifications / Licensure:

• At least 1 year of experience in a Medicare-certified home health agency.
• Active and clear driver's license and car insurance.
• Valid Hawaii RN license.
• Experience with Homecare Home Base and OASIS.
• COVID-19 and FLU vaccinations are required.
• Bachelor's degree in nursing is preferred.
• Previous case management experience is preferred.
