Job Description
Physical Therapist (PT)

We are seeking a dedicated and experienced Physical Therapist to provide exceptional care in the home health setting. This position requires a strong commitment to patient health and a passion for rehabilitation.

Responsibilities

Conduct initial evaluations, reassessments, and develop individualized treatment plans for patients.
Administer therapy treatments and modalities specific to patient needs.
Document patient progress and modify treatments as necessary.
Collaborate with a clinical manager or educator for ongoing support.
Ensure compliance with all regulatory standards and best practices.

Patient Demographics

Common diagnoses include Congestive Heart Failure (CHF), orthopedic conditions, and pneumonia.
Average patient caseload: approximately 6.5 patients per day.

Qualifications

General Certification Requirements: Basic Life Support (BLS).
Minimum of 1 year of relevant experience is preferred; no new graduates, please.

Work Schedule

Shifts consist of 5 eight-hour days, from 8:00 AM to 5:00 PM.
Rotating weekends required.
Holiday rotation may be necessary.

Orientation and Support

Comprehensive orientation lasting 2 weeks with an assigned preceptor.
Access to a clinical manager or educator for support and guidance available 24/7.

Dress Code

Employees are required to wear scrubs or business casual attire while on duty.

Additional Information

Mobile devices, including phones and tablets, will be provided for work purposes.
