About the Role As a Licensed Practical Nurse (LPN) with Axiom Gardens of Flora, you will play a vital role in our healthcare team, providing exceptional care to our residents in a fast-paced and dynamic environment. You will be responsible for: Providing direct and indirect patient care, including administering medications, performing vital signs, and maintaining accurate records Assisting with daily living activities, such as bathing, dressing, and grooming Collaborating with other healthcare professionals to develop and implement individualized care plans Identifying and reporting changes in resident condition to the nurse supervisor and other healthcare professionals Maintaining a clean, safe, and organized environment in the resident's living space Responsibilities Conduct initial and ongoing assessments of resident needs, and develop and implement individualized care plans Monitor and report changes in resident condition to the nurse supervisor and other healthcare professionals Provide emotional support and companionship to residents and their families Participate in interdisciplinary team meetings to discuss resident care and progress Maintain accurate and up-to-date records of resident care and services Collaborate with other healthcare professionals to provide comprehensive and coordinated care Requirements Must be a Licensed Practical Nurse (LPN) in the state of Illinois #J-18808-Ljbffr Axiom Care
