Responsibilities Conduct comprehensive assessments and on-going re‑assessments of clients, including psychosocial, physical, mental health, environmental, and spiritual needs. Write comprehensive assessments and develop, initiate, and monitor the Community Living Plan in accordance with program guidelines and policies. Conduct home visits, hospice and skilled nursing facility visits, and escort clients to medical appointments as clinically indicated. Identify, arrange, and monitor appropriate community services using knowledge of Medicare, Medi‑Cal, and other entitlement programs. Establish and maintain a care‑management relationship with clients and their significant others, offering respect, dignity, and support. Provide crisis intervention, advocacy, problem‑solving, and therapeutic interventions. Meet with clients at least monthly, and more often as clinically indicated, reviewing and modifying their care plan on an ongoing basis. Document all case‑management activity in progress notes within 24–48hours, adding new problems to the care plan as needed. Maintain required paperwork and follow a consistent charting system to ensure continuity of care. Evaluate client purchase‑of‑service needs, ensuring timely delivery of services. Educate clients and significant others about resources and services. Communicate effectively with community providers, including physicians and other health‑care and social‑service workers. Monitor the quantity and quality of services provided by other providers and work with the team to evaluate clients’ ability to remain safely at home. Coordinate placement as appropriate according to Community Living Fund guidelines; collaborate with the client, caregiver, and involved services to terminate clients when appropriate. Participate in data collection and Continuous Quality Improvement efforts. Attend and actively participate in team and program meetings, contributing to open lines of communication and providing updates on caseload activity. Utilize supervision appropriately and maintain open communication with supervisors. Actively apply ethical and legal standards of the National Association of Social Workers and regulatory requirements of the Community Living Fund. All other reasonably related responsibilities as assigned. Education B.A. or B.S. in Social Work or a related field required. Background and Experience One year of experience working with disabled adults and/or older adults. Understanding of medical and psychosocial problems of functionally impaired adults and older adults. Experience working with individuals with mental and/or behavioral health diagnoses and substance‑abuse disorders highly desired. Exceptional communication and presentation skills linking clients, support systems, and health‑professional teams. Demonstrated case‑management skills and experience in the community health‑care delivery system. Detail‑oriented with strong problem‑solving skills and ability to prioritize multiple tasks. Computer literacy required. Bilingual English/Spanish required. Compensation Range: $72,000 to $74,000 per year. This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education, and other factors.
