Outpatient Social Worker - Care Coordination & Resources

SEARHC is seeking a licensed Master Social Worker to join the Social Services team in Sitka, AK. The role focuses on care coordination, resource navigation, and outpatient psychosocial support across patient ages from birth to elder. You will work within an interdisciplinary treatment team, maintain EMR documentation, and advocate for access to community resources and benefits.
