Benefits Earn $85 - $95 per visit Competitive, weekly pay with travel time and mileage reimbursement Holiday and PTO pay for full-time employees Supportive, caring management that will have your back Medical for full-time, dental, vision, and supplemental benefits available for everyone 1:1 patient ratio; where you make a difference in our patients’ care with flexible assignments to fit your needs Continuing education benefits and discounted courses through Colorado Christian University and Rasmussen University Responsibilities Provide home-based social services to patients dealing with depression, anxiety, poor nutrition, or lack of resources that make managing their injury or disease difficult Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT, OT, or SLP, focused on the patient’s plan of care and individual goals Monitor a patient’s psychosocial condition and identify social and emotional needs Conduct patient assessments, document progress, and ensure patients are moving toward goals Provide counseling, community resource planning, crisis intervention, and advocacy Consult with family and caregivers on the patient’s plan of care and how to support progress Requirements Master’s degree in Social Work and active LCSW license in Arizona Minimum of one (1) year of social work experience, ideally in home healthcare CPR Certification (demonstration course required) Valid Arizona Fingerprint Card Knowledge of state and federal home health regulations Strong interpersonal skills; empathetic, compassionate, resourceful, and an effective communicator Why Work for Interim HealthCare?
