Work Environment

This position is an onsite role, primarily within an assisted living facility located in Rotterdam Junction, New York. Key Responsibilities

• Conduct comprehensive health assessments, including cardiovascular screenings, to identify acute and chronic conditions in residents.
• Manage patient care plans, ensuring continuity by coordinating referrals for specialized services as needed.
• Participate in community outreach initiatives such as health fairs and wellness seminars to promote cardiovascular health awareness.
• Document all patient interactions meticulously, maintaining compliance with regulatory standards and ensuring accurate medical records.

Patient Population / Client Focus

Qualifications

• A Master’s degree in Nursing from an accredited program.
• Active nursing licenses in the state of New York.
• Current Nurse Practitioner certification.
• A minimum of three years of clinical experience, preferably in community health, public health, or cardiology settings.

Compensation and Benefits
