Design individualized discharge pathways that integrate patient values and family preferences, ensuring a safe and sustainable transition from the hospital to the next stage of care. Advocate for vital patient resources by identifying and removing systemic barriers to care, ensuring every individual has the medical necessity and support required for their specific diagnosis. Evaluate clinical documentation accuracy to provide expert recommendations on levels of care, ensuring that medical records reflect the true complexity of the patient's journey and treatment options. Drive continuous care improvement through active collaboration with the care transition team, constantly re-evaluating health plans to adapt to a patient’s changing condition in real-time. What minimum requirements you’ll need Licensure / Certification / Registration: Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required. BLS Provider preferred. American Heart Association or American Red Cross accepted. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire. What additional preferences we're seeking Case Management Expertise: A minimum of 3 years of dedicated experience in a Case Management role within a healthcare setting. Discharge Planning Expertise: Demonstrated experience in coordinating complex patient transitions and multidisciplinary care plans.
