## Clinical Pharmacist - Surgical Intensive Care and General SurgeryApplyremote type: No Teleworklocations: Seattle, WAtime type: Full timeposted on: Posted Todayjob requisition id: REQ-0000134675**Job Description**The **UW Medical Center-Montlake Pharmacy Department** is seeking a highly-motivated**Clinical Pharmacist - Surgical Intensive Care and General Surgery.****WORK SCHEDULE** • Full-Time / 40 Hours Per Week • Rotating/Variable Shift**POSITION HIGHLIGHTS*** Our Clinical Pharmacists provide extensive pharmaceutical care and are a vital part of the inpatient multidisciplinary patient care team.* In addition to direct patient care responsibilities, our pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care and precept pharmacy students and residents.* Applicants should have an advanced degree, or residency, or two years of experience in clinically-oriented hospital pharmacy practice. PGY-2 critical care training preferred**PRIMARY JOB RESPONSIBILITIES*** Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.* Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.* Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.* Provide drug information and education to medical teams, nursing, and dietitian staff.* Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders at each visit.* Provide primary preceptorship of pharmacy residents and student pharmacists.* Perform duties of a Pharmacist 2.**REQUIRED POSITION QUALIFICATIONS**Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.**LEGAL REQUIREMENT**State of Washington Pharmacist License.*CONDITION OF EMPLOYMENT**UW Medicine is committed to being a drug-free workplace.
