Job Description
Title: CNA - Rehabilitation

Schedule: 12-Hour Day Shift

Assignment Details:
Contract Length: 6 weeks
Guaranteed Hours: Not specified

Requirements:
- Must have a valid license on file at the time of submission
- BLS Certification through AHA required
- Minimum of 3 years of experience required
- Prior travel experience required
- Long-term care (LTC) experience required
- Ability to float to other units as needed
- Comfort working with dementia patients

Roles & Responsibilities:
- Provide direct patient care and support daily living activities.
- Assist in the development and implementation of patient care plans.
- Monitor and document patient behaviors and health status.
- Maintain a safe and clean environment for patients.
- Communicate effectively with patients, families, and healthcare team members.
- Participate in training and continuing education as required.

Compliance / Notes:
- A 7-year background check (nationwide, county court searches, FACIS3, and sex offender) is required within 18 months of the start date.
- FACIS 3 and VCIC must be completed within 30 days.
- All employment gaps over 90 days must be explained.
- All work history information (bed count, EMR, facility type, etc.) must be complete to submit.
- Public transportation is not accessible; reliable transportation is recommended.
- Vehicles should be equipped with all-season or snow tires during the months of November to April.
- Requests for time off once the assignment begins will not be honored.
