This is done utilizing the nursing process in accordance with hospital standards and unit policies/procedures. The purpose of this is to monitor and evaluate patient functional status. Utilizes the nursing process to care for patients. Assesses, organizes, and completes patient care with consideration for patient age and psychosocial factors. Documents steps of the nursing process. Utilizes proper medical terminology and addresses problems and progress toward goals. Facilitates patient discharge through timely, appropriate referrals. Utilizes patient education resources suitable to patient age and psychosocial factors. Collaborates with other disciplines as appropriate. Supports unit and hospital goals through performance improvement and educational activities. Attends in-services and meets education standards. Completes variance reports as required. Utilizes independent judgment in making decisions from various alternatives. Coordinates activities of others but does not supervise them. Leads others in the same work performed but does not supervise. Has input on performance evaluations but does not prepare or give evaluations. Works directly with patients and customers. Engages with internal customers via telephone or face-to-face interactions. Interacts with external customers via telephone or face-to-face interactions. Works with other healthcare professionals and staff. Frequently collaborates with individuals at a director level or above.
