Licensed Practical Nurse, Centro De Salud

The Licensed Practical Nurse (LPN) provides care to designated patients within the Pennsylvania Licensed Practical Nurse Scope of Practice. Provides care coordination across the continuum. Participates in Network quality improvement initiatives. Demonstrates annual competence with clinical skills, clinical equipment, and electronic medical record system. Performs administrative functions as necessary.

Education:

• Graduate of an accredited Licensed Practical Nursing Program
• Bachelor's degree in a health science, or a related field, preferred.

Experience:

• 1 2 years of experience working in an office based medical clinic, preferred.
• Bilingual English/Spanish required

Licensure:

• LPN license in Pennsylvania
• Basic Life Support (BLS)
