Physical Therapist (PT)

Role Provides direct physical therapy care to patients in a skilled nursing facility setting, focusing on long-term care and short-term rehabilitation. Responsibilities Develop, assess, and modify treatment plans to meet patient goals. Supervise and evaluate therapy assistants, aides, and students. Maintain accurate, timely documentation and utilize therapy software. Collaborate with interdisciplinary teams to ensure comprehensive patient care. Assist in patient evacuation and deliver excellent customer service. Follow safety protocols, confidentiality policies, and company standards. Qualifications & Skills Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT); licensure in applicable state. Preferably 1+ year of post-acute care experience. Current CPR certification; proficient in Microsoft Word, Excel, and email. Strong communication, reasoning, and teamwork skills. Ability to perform physical activities including lifting and moving patients.
