About Solara Hospice Solara Hospice is a mission-driven hospice organization serving patients and families in Eastern Massachusetts. Position Overview The Hospice RN Case Manager is responsible for coordinating and delivering comprehensive nursing care to hospice patients. This role supports patients and families throughout the end-of-life process.
