Now Hiring: Full-Time Physical Therapist (PT) Home Health Camden, TN & Surrounding Counties Schedule: MondayFriday | 8:00 AM 4:30 PM- Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation. Competitive pay 401(k) with generous company match Paid holidays and generous paid time off Employee referral program Merit-based pay increases Employee discount programs Supportive team culture and healthy work-life balance Key Responsibilities Evaluate patients and develop individualized treatment plans based on physician orders Provide skilled physical therapy services in accordance with the physician’s plan of care and state regulations Help patients improve mobility, strength, balance, and functional independence Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques Recommend adaptive equipment and home modifications to promote safety and independence Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes Complete accurate and timely clinical documentation and care plan updates Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations Encourage and motivate patients throughout their rehabilitation journey Qualifications Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) Current Physical Therapist (PT) license in the state of Tennessee Home Health experience preferred, but not required Two (2) years of experience in an acute care or similar clinical setting preferred Current CPR and First Aid certification Valid driver’s license, reliable transportation, and proof of automobile insurance #J-18808-Ljbffr Addus HomeCare
