Geriatric Nurse Practitioner — Primary Care & Home Visits
PACE
PACE SEMI in Michigan is seeking a Nurse Practitioner to provide primary care for an assigned participant panel. The NP will manage acute and chronic care under supervision of the center PIC, perform regular assessments, and coordinate care with the interdisciplinary team. Responsibilities include chronic disease management, pharmacologic prescribing delegated by the physician, preventive care, and documentation in SOAP format within 24–72 hours. #J-18808-Ljbffr PACE
