(Only QUALIFIED Healthcare Professionals accepted) Travel NP - Large physician-owned multispecialty group is seeking a Nurse Practitioner to participate in an innovative home-health program designed to provide coordinated care in a team environment that includes a supervising physician and support staff

Nurse Practitioner (NP) will become an important member of a team that would provide initial patient assessment, devise and implement a care plan, and mobilize other nursing, therapy, and community services.


Most the patients will be homebound, but many will be recently-discharged from a hospital or nursing facility, or require closer care than can be provided by a visiting nurse.

The Nurse Practitioner would reconcile medications, educate patients and caretakers, order testing as appropriate, and communicate closely with visiting nurse agencies, primary care physicians, and specialists.


Patients will be referred by PCPs, hospitals, and case managers. Daily visits would most often be performed by the visiting nurses, with the nurse practitioner, primary care provider (PCP), and medical director available as backup resources.


Compensation will be competitive, with benefits that include health insurance, malpractice coverage, a 401K retirement plan, a CME allowance, and vacation.
