• Faith Hospice: end of life care in our inpatient residence called Trillium Woods as well as in the community
• Reliance Community Care Partners: connecting older adults with health care resources in the community

We have the following RN opportunities available to explore with you:

• Holland Home skilled nursing: part-time or full-time roles on 1st, 2nd, or 3rd shift available
• Atrio Home Care:
  • RN Case Manager - med/surg or behavioral health
  • RN Intake
  • RN private duty home visits
• Care Resources PACE: per diem RN Home Care Coordinator
• Faith Hospice:
  • RN Case Manager
  • RN Response Nurse - after hours & weekends
  • RN Inpatient Hospice: 2nd or 3rd shift
