Responsibilities

• Provide direct patient care to residents within the licensed practical nurse scope of practice
• Administer medications and treatments per physician orders and facility policy, document administration accurately
• Monitor residents’ conditions, document changes, and report to the RN or charge nurse
• Assist with activities of daily living, promote independence, and uphold resident dignity and comfort
• Maintain a safe, clean, and therapeutic environment; enforce infection control and safety protocols
• Collaborate with the interdisciplinary team to support care plans and resident outcomes

Requirements

• Active LPN license in Vermont (or eligibility for licensure in Vermont)
• Current Basic Life Support (BLS) certification
• Relevant experience in long-term care or skilled nursing settings preferred
• Graduate of an approved practical nursing program
• Ability to work 40 hours per week and adapt to day and/or evening shifts in a travel assignment
