Towne U.S. LLC is seeking a travel Physical Therapist for a travel job in Blackwell, Oklahoma. Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel We are seeking an experienced Travel Physical Therapist (PT) for a 13-week Home Health assignment . The PT will provide skilled therapy services to patients in their homes and residential settings, helping them improve mobility, reduce pain, restore function, and maximize independence. This position is ideal for a therapist who enjoys working autonomously while delivering high-quality, patient-centered care in the home health setting. Assignment Details Length: 13 Weeks Schedule: Days, Monday–Friday Missed visits may be made up on weekends as needed Guaranteed Hours: 40 hours per week Travel Radius: Up to 50 miles from the facility Expected Weekly Mileage: Approximately 50–75 miles (may vary) Mileage: Included in bill rate Patient Load: Approximately 6–7 visits per day Charting System: WellSky Required Daily Stand-Up Meeting: Monday–Friday at 8:30 AM Responsibilities Evaluate patients and develop individualized physical therapy treatment plans Provide therapeutic exercises, gait training, balance training, manual therapy, and functional mobility interventions Monitor patient progress and modify treatment plans as needed Educate patients and caregivers on home exercise programs, safety strategies, and recovery plans Complete accurate and timely documentation in WellSky Coordinate care with physicians, nurses, therapists, and other healthcare professionals Participate in interdisciplinary care planning to ensure optimal patient outcomes Deliver compassionate, patient-centered care in home and residential settings Requirements Required: Graduate of an accredited Physical Therapy program Required: Active PT license in the state of assignment or ability to obtain Required: Current BLS and First Aid certifications Required: Minimum 2 years of recent licensed PT experience Required: At least 1 recently completed travel assignment Required: Experience with HomeCare/HomeBase Required: Home Health experience No New Graduates Additional Requirements COVID-19 vaccination and booster(s) required Medical and religious exemptions considered on a case-by-case basis Approved exemptions require weekly testing per facility policy Candidates should review housing options prior to submission What We Offer Competitive weekly pay Comprehensive benefits package Dedicated recruiter support throughout the assignment Opportunity to work independently while making a meaningful impact on patients in their homes Potential for assignment extensions and future travel opportunities Towne U.S. LLC Job ID 18532924.
