Learn more at About the Role The Patient Navigator position is an exciting opportunity for compassionate, service‑minded individuals who want to make a meaningful impact on patients' lives. In this role, you’ll serve as the primary point of contact for patients, guiding them through their fertility journey and working closely with a multidisciplinary team to ensure exceptional care and support. We are seeking a Patient Navigator to join our dedicated team in Pinnacle Fertility – Arizona, in Chandler or Glendale, AZ. This is a full‑time position, working Monday – Friday 7:30am – 4:30pm. Training and travel will be required in Scottsdale, AZ for the first 12 weeks. Responsibilities Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow‑up care. Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care. Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination. Oversee the treatment coordination between departments for individual patient care plans working closely with financial, embryology, and genetics. Maintain accurate and up‑to‑date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system. Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care. Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery. Educate patients on treatment protocols, self‑care strategies, and medication usage. Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans. Advocate for patients’ needs and help resolve any issues or barriers to care. Assist with after‑hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately. Other duties and projects as assigned. Position Requirements Education: High school diploma required. Bachelor’s degree in healthcare, social work, or a related field preferred; Certified Medical Assistant Certification (CMA) preferred. Experience: Minimum of 2 years in customer service, patient navigation, case management, or a related healthcare role. Skills: Strong knowledge of healthcare systems, insurance processes, and patient advocacy. Excellent communication, organizational, and problem‑solving skills. Proficiency in using electronic medical records (EMR) and healthcare software. Empathy and a patient‑focused approach, with the ability to work collaboratively in a multidisciplinary team. Third‑Party Reproduction experience highly preferred; FDA tissue regulations (21 CFR Part 1271) experience preferred. Bilingual skills are a plus. Hourly Rate: $22.00 – $26.00 per hour (Final offers based on experience, skills, and qualifications). Benefits: Comprehensive healthcare, dental, life, and vision insurance.
