Registered Nurse (RN) General Purpose The primary responsibility of the registered nurse in this position is to supervise the day‑to‑day nursing activities of the facility during the assigned tour of duty, ensuring compliance with federal, state, and local standards, guidelines and regulations, and maintaining the highest quality of care at all times. Essential Duties Those listed below are essential functions of the position. The list is not exhaustive; the RN may be required to perform additional tasks that are related to or essential to the role. Administrative Functions Assist the Director of Nursing Services in directing day‑to‑day nursing activities. Participate in developing, maintaining, and updating written policies and procedures that govern nursing service functions. Ensure reference materials at nurses’ stations are current. Recommend written materials to support nursing service needs. Maintain the Nursing Service Procedures Manual in alignment with daily nursing procedures. Enforce compliance with the procedures in the Manual by all nursing personnel. Prepare written and oral reports/recommendations to the Director regarding departmental operations. Review policies, procedure manuals, and job descriptions periodically, proposing revisions to the Director. Coordinate nursing services with other resident services to ensure continuity of care. Ensure personnel compliance with their respective job descriptions. Develop, maintain, and implement the facility’s quality assurance program for the nursing service department. Participate in facility surveys and inspections conducted by authorized government agencies. Review and update resident discharge plans as required. Assist the Director in planning nursing services for resident discharge. Meet with nursing staff and support personnel to plan departmental services, programs, and activities. Admit, transfer, and discharge residents as needed. Complete accident and incident reports. Interpret department policies and procedures for personnel, residents, visitors, and government agencies. Perform administrative duties such as completing medical forms, reports, evaluations, and charting. Maintain confidentiality of user ID codes and passwords, reporting violations to the Administrator. Report unauthorized attempts to access the facility’s information system. Committee Functions Attend committee meetings (Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment, etc.) as required. Assist in preparing written and oral reports of nursing programs for these committees. Provide information to the Quality Assurance and Assessment Committee upon request. Help develop agendas for the Care Plan Committee meetings. Participate in reviews of discharge plans and provide them to the Care Plan Committee as needed. Implement committee recommendations in accordance with the Director’s direction. Personnel Functions Ensure sufficient licensed practical and registered nurses are available during the tour of duty. Ensure a sufficient number of certified nursing assistants are available to provide routine nursing care. Develop work assignments and assist in carrying them out. Delegate administrative authority and accountability to each Charge Nurse. Ensure nursing assistants are enrolled in or have graduated from an approved training program. Ensure nurse aide trainees are supervised by a licensed nurse. Conduct daily rounds to verify assignments are performed to nursing standards. Report findings to the Director. Monitor absenteeism to maintain adequate staffing. Report problem areas to the Director. Enforce dress code and other departmental policies. Review complaints and grievances filed by personnel. Make reports to the Director as required. Build rapport with inter‑department personnel and other departments. Maintain a calm, positive environment throughout the unit and shift. Meet shift regularly to address problem areas and improve services. Evaluate the department workforce and recommend improvements. Participate in employee performance evaluations and staffing decisions. Ensure disciplinary actions are fair and non‑discriminatory. Report occupational exposures to blood, body fluids, infectious materials, hazardous chemicals. Report incidents of fraud to the Administrator. Ensure unattended computer workstations are properly locked or password protected. Nursing Care Functions Notify nursing personnel of new admissions, expected arrival, room assignment, and other details. Prepare rooms for new admissions. Greet newly admitted residents. Escort residents to rooms as necessary. Make rounds with physicians as needed. Encourage physicians to record and sign progress notes, orders promptly. Inform Director if physician visits are not timely. Consult with residents, families, and physicians to plan care and treatment. Notify attending physician and next‑of‑kin of changes in resident condition. Visit residents daily to assess physical and emotional status. Ensure direct nursing care is provided by qualified personnel. Review nurses’ notes for completeness and accuracy. Provide direct nursing care as needed. Start IVs, obtain sputum, urine and other specimens for lab tests. Take temperature, pulse, respiration, and blood pressures as required. Admit, transfer, and discharge residents as needed. Report problems to the Director. Assist in developing and implementing corrective action. Keep the Director informed of resident status and related matters by written or verbal reports. Assist the Charge Nurse in monitoring seriously ill residents. Authorize restraints when necessary per facility policies. Ensure compliance with medication administration, storage, and control procedures. Monitor medication pass and treatment schedules to verify accurate administration. Review medication cards for completeness and accuracy. Inspect treatment areas daily for cleanliness and safety. Check residents who cannot call for help frequently. Meet with residents and families as necessary. Report problems to the Director. Assist in arranging transportation for discharged residents. Inform families of resident death. Call funeral homes when requested by family. Staff Development Participate in developing, planning, conducting, and scheduling in‑service training classes to instruct nursing staff. Assist the In‑service Director in developing annual training programs (OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control, etc.). Assist the Director in planning clinical supervision for nurse aide trainees. Attend continuing education programs to maintain professional competence. Safety and Sanitation Assist the Safety Officer in developing safety standards for the nursing service department. Assist in identifying and documenting tasks with potential exposure to blood or body fluids. Ensure personnel receive appropriate in‑service training before performing exposure tasks. Ensure adequate supply of personal protective equipment is available. Enforce proper use and disposal of PPE. Monitor shift personnel for compliance with safety regulations. Maintain clean and sanitary work areas and resident care rooms. Encourage handwashing and hygiene procedures. Participate in fire safety and disaster preparedness drills. Develop, implement, and maintain infection control program for communicable diseases. Ensure isolation precautions are followed when necessary. Develop procedures for reporting hazardous conditions or equipment. Ensure safety equipment (back brace, mechanical lifts) is used when moving residents. Report missing or illegible labels and MSDS to Safety Officer. Equipment and Supply Functions Recommend equipment and supply needs to the Director. Maintain adequate stock of medications, supplies, and equipment. Assist Maintenance Director with the preventive maintenance program for department equipment. Develop and implement safe operation procedures for nursing equipment. Ensure only trained personnel operate equipment. Monitor use of supplies to prevent waste. Ensure hazardous chemical MSDSs are on file. Care Plan and Assessment Functions Develop written assessments of nursing needs for each resident. Create comprehensive care plans identifying problems, care, goals, and responsible professionals. Encourage resident participation in care plan development. Review care plans for appropriate goals and revisions. Ensure all staff are aware of and follow the care plan. Review nurses’ notes to confirm care plan adherence. Assist the Resident Assessment Coordinator with MDS planning and revisions. Confirm medical and nursing treatments align with resident wishes and care plan. Budget and Planning Functions Monitor economic conditions and recommend adjustments to nursing services. Report suspected or known fraud incidents involving billing, cost reports, or kickbacks. Supervisory Requirements Assist with the overall supervision and management of nursing staff. The RN is delegated administrative authority, responsibility, and accountability necessary for carrying out assigned duties. Qualification – Education and Experience Possess a nursing degree from an accredited college or university. Have training in rehabilitative and restorative nursing practices. Have at least two years of experience, preferably in long‑term care. Language Skills Read, write, speak, and understand English. Read technical procedures. Read and comprehend policy and procedure manuals. Present information effectively and respond to managerial and employee questions. Mathematical Skills Apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Reasoning Ability Make independent decisions when circumstances warrant. Solve practical problems. Tactfully deal with personnel, residents, families, visitors, government agencies, and the public. Provide leadership and supervise effectively, working harmoniously with others. Plan, organize, develop, implement, and interpret programs, goals, and policies for quality care. Demonstrate knowledge and skills necessary for age‑related resident care. Support teamwork and demonstrate good teamwork principles. Understand nursing, medical practices, laws, regulations, and guidelines. Display patience, tact, cheerfulness, enthusiasm, and willingness to handle difficult residents. Seek new methods and incorporate them into practice. Relate information regarding resident condition effectively. Maintain safety and health of others. Certificates, Licenses, Registrations Current, unencumbered, active license to practice as an RN in the state. Current CPR certification. PCC knowledge and experience preferred. Maintain required continuing education and licensing. Remain in good standing with the State Board of Nursing. Physical Demands Move intermittently throughout the workday. Speak and write in English understandably. Cope with mental and emotional stress. Use vision, hearing, or prosthetics to function effectively. Function independently, flexibly, and with integrity. Meet general health requirements, including medical and physical examinations. Work with ill, disabled, elderly, emotionally upset, or hostile patients. Push, pull, move, or carry at least 25 pounds to a minimum of 5 feet and 50 feet. Assist in evacuation during emergencies, when necessary. Work Environment Office areas and nursing service areas (drug rooms, nurses stations, resident rooms). Move intermittently during work hours. Frequent interruptions. Interact with residents, personnel, visitors, government agencies. Encounter hostile or upset residents, families, personnel, and visitors. Communicate with medical staff and department directors. Work beyond normal hours, weekends, and holidays as needed. Possible call back during emergencies (severe weather, evacuation, post‑disaster). Participate in community and civic health projects. Attend continuing education programs. Exposure to falls, burns, odors, disinfectants, tobacco smoke, and other air contaminants. Exposure to infectious waste, diseases (TB, AIDS, hepatitis B). Potential exposure to hazardous chemicals. Maintain liaison with residents, families, support departments for resident needs.
