Travel Physical Therapist (PT) - $1,993 per week in Aiken, SC

Key Responsibilities
Conduct home health visits including Start of Care and Follow-Up visits, seeing an expected 5-6 patients per day
Travel to patient homes and care facilities within the assigned territory, covering up to 75 miles from the home clinic
Meet weekly productivity expectations of 30 points per week, equating to approximately 40 hours per week
Complete all clinical documentation within 24 hours of each patient appointment using

HomeCare HomeBase EMR

and OASIS
Attend periodic office/clinic meetings as required
Accept all scheduled patients regardless of distance from previous appointment, in alignment with program expectations
Required Qualifications
Active, unencumbered Physical Therapist license in hand - no pending licenses accepted
Minimum 1 full year of recent Adult Home Health experience

• first-time home health candidates will not be considered
  CPR/BLS certification required
  Must have a reliable personal vehicle, valid driver's license, and agency-required auto liability insurance
  Must be proficient in HomeCare HomeBase EMR within the last 6 months, or willing to complete a retraining course during compliance
  Locals are welcome and accepted at the same rate as travelers
  Preferred Experience / Skills
  Experience with OASIS documentation used within the last 6 months (retraining course available if needed)
  Comfort with extended drives between patients; mileage radius may result in drives longer than 20 minutes between appointments
  Familiarity with a point-based productivity model in a home health setting
  Flexibility with schedule variations, including potential Baylor shift (4x10s, Friday-Monday) depending on territory
