Dialysis Care Navigator - Outpatient RN (Regional Float)

Fresenius Medical Care

Fresenius Medical Care is seeking an Outpatient RN CAP 2 for the End Stage Kidney Disease care team. This role focuses on assessment, planning, intervention, and evaluation for an assigned patient group, with opportunities to mentor staff and lead quality initiatives. Responsibilities include data collection during dialysis, care plan updates with the team, patient education, and adherence to policies.
