Job TitleRN (Registered Nurse), Home HealthCity/State:Elizabethtown, KYShift:8H Days M-F, 8a-5p + weekend rotationContract Length:13 weeksStart Date:08/03/2026Job Description:The RN in Home Health is responsible for delivering care to patients across multiple counties, utilizing Homecare Homebase for charting. The role requires experience in home health and proficiency with Homecare Homebase charting. The RN will manage approximately 30 adult patients, ensuring start of care, recerts, and daily visits according to the point system.Responsibilities:Provide care to patients in a home health setting across specified countiesUtilize Homecare Homebase charting systemCase manage approximately 30 patientsEnsure completion of 6.5 points per day, including start of care and recertsRequirements:Valid RN License in the State of workMinimum of 1 year of experience on a Medsurg floor in a hospital settingExperience with Homecare Homebase chartingBLS Certification
