AdventHealth Medical Group is seeking a compassionate and collaborative Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our established Palliative Care team at AdventHealth New Smyrna Beach in New Smyrna Beach, Florida. Position Highlights

• Join an established hospital-based Palliative Care program
• MondayFriday schedule, 8:00 AM 5:00 PM
• No procedures required
• Collaborative team environment with experienced physicians and clinical staff
• Opportunity to care for patients with complex medical conditions while focusing on symptom management, goals of care, and quality of life

Responsibilities

• Provide comprehensive palliative care consultations and follow-up care for hospitalized patients
• Collaborate with physicians, nurses, case management, social work, chaplain services, and other interdisciplinary team members
• Assist patients and families with goals-of-care discussions, advance care planning, and symptom management
• Develop individualized treatment plans that align with patient values and preferences
• Document patient care accurately within the electronic medical record
• Participate in multidisciplinary rounds and care coordination

• Licensed Nurse Practitioner or Physician Assistant (or eligible for Florida licensure)
• Board Certified
• Interest or experience in Palliative Care and/or Hospice Medicine
• New graduates are welcome to apply if they demonstrate a strong interest in palliative medicine
• Excellent communication and interpersonal skills
• Ability to work collaboratively in a multidisciplinary team
